Fill PDF Form reads the interactive fields built into a PDF (the AcroForm fields that Acrobat, Word and most form builders create) and shows each one as a normal web form: text boxes and multi-line boxes, checkboxes, radio buttons, dropdowns and selection lists. Fields are grouped by page in reading order and use the form's own tooltips as labels where the PDF has them. Read-only fields are shown but can't be changed.
When you press Fill and download, the values are written into the same fields with pdf-lib, so the result is still a fillable form you can correct later. Tick Flatten after filling and the answers become part of the page instead, so nobody can edit them; that is what many offices ask for when you email a completed form.
Limits: flat or scanned forms have no fields to read, so this tool says so and points you to Edit PDF, where you can type anywhere on the page. Dynamic XFA forms made in Adobe LiveCycle can't be filled in a browser. Signature fields are listed but not signed here. Answers with characters the built-in PDF font can't draw (such as Hindi or Chinese) are saved as field values for the reader to display, and can't be flattened.
